The first step is to simply call us, text or feel free to book online.

From there we will verify your insurance with a complimentary benefit check and get you scheduled as a new patient. You can fill out your intake forms on-line (we will send you a link via email).
Insurance Coverage
We accept most insurance plans and are always eager to help our patients navigate and understand their insurance benefits.
The companies that can be billed directly by us if your coverage permits it are listed below. Most insurance plans have coverage for chiropractic care. Please check with your provider to determine your level of benefits. Most patients find chiropractic care to be extremely cost-effective and affordable.
Providence, Blue Cross Blue Shield (BCBS), Moda, PacificSource, out of network benefits available for United Health Care (UHC)
Some insurers may require you to have authorization from them before you begin. If necessary, we will obtain this authorization for you once we have your insurance information.

New Patient Forms
If this is your first visit to our clinic, we will ask you to complete a Patient Registration Form, a Medical History, and sign the Consent Form.
Once you schedule, we will reach out with a link that will allow you to fill your intake form online.
